Methotrexate Treatment of Unruptured Ectopic Tubal Pregnancy

Abstract
16 cases of nuruptured tubal pregnancy were treated with four doses of intramuscular methotrexate(1.0mg/kg)followed by four doses of intramuscular leucovorin (0.1mg/kg).
The diagnosis was established serial check of serum ¥â-hCG, endometrial curettage with biopsy, serial check of vaginal ultrasonography and diagnostic laparoscopy.
Subjects were followed weekly quantitative serum ¥â-hCG radioimmunoassay and vaginal sonography.
The procedure was successful in 13 cases(76.4%), as indicated by reduction of serum ¥â-hCG value to 5 mIU/L.
The remaining 4 cases showed plateau or increased in hCG after methotrexate therapy indicating the persistence of trophoblastic function. So, four subjects underwent salpingectomy or salpingoophorectomy following treatment.
It is concluded that methotrexate treatment represents an attractive method for nom surgical treatment of unruptured ectopic tubal pregnancy, preferencially in subjects with low trophoblastic activity.
